FIVEFOLD MINISTRY QUESTIONNAIRE – DISCOVER YOUR MINISTRY GIFT!

We would love to help you discover your Fivefold ministry gift. Our survey is designed to give you a snapshot of your Base and Phase ministry gifts as articulated in Ephesians 4 (Apostle, Prophet, Teacher, Evangelist, Pastor). Please complete our free test that measures how you are wired by assessing how your motivations align with the five gifts. Try to ‘go’ with what your gut says first when you answer these questions.

You can complete the Fivefold questionnaire in one of three ways:

1. Print out the PDF Questionnaire and complete it. Click here.
2. Open and complete the Attached Excel spreadsheet. Fill in your answers on the Questions tab with a 1 (number 1), and the second Tab will automatically give your ranking by colour shading – green is the highest, red is the lowest. Click here.
